Insurance

I am in network with most major insurances including:

  • Medica
  • Blue Cross Blue Shield
  • Preferred One
  • UCare
  • Medical Assistance
  • Medicare

For people using insurance I will charge the following:

Initial appointment/Diagnostic Assessment 200$  (CPT code 90791)
Therapy appointments 53-60min 175$   (CPT code 90837)

Please note that what you pay and what your insurance actually reimburses is dependent on your insurance policy.

If you choose not to use insurance and instead pay up front, my rate is 150$/session.

For payment I accept cash or check.  I do not accept credit card payments.

We can discuss other options around payment as needed.
